Most problem solving, failure
analysis methods focus on a single cause
which is typically referred to
as the "root cause." The Cause
Mapping
method of root cause analysis
demonstrates how problems occur as a
system of causes that reveals
different ways to solve the problem. The
simplest, most effective
solutions may be a combination of solutions to
mitigate the risk to an
acceptable level. This presentation
provides
the basics of thorough
cause-and-effect analysis and explains the
fundamental role of
evidence. Tips for documenting and
communicating
complex problems will be
provided when using paper, dry-erase boards and
for creating detailed visual
Cause Maps within Microsoft Excel.
